All forms for insurance can be found here, please be sure to CC the League President and Vice-President in any emails to CARHA insurance to ensure that they are aware of the claim and able to assist if required. Be aware that all claims require a physician visit within 30 days of the injury and treatment within 90 days otherwise the claim may not be reviewed by insurance.
